Did you know…

Immunizations are an important part of our children’s and the community’s health. Colorado law requires students attending a public, private or parochial school to be immunized against certain vaccine-preventable diseases. Colorado follows the Centers for Disease Control and Prevention’s Advisory Committee on Immunization Practices schedule. This schedule is approved by the American Academy of Pediatrics, the American Academy of Family Physicians and the American College of Obstetricians and Gynecologists. This is the immunization schedule which will best protect your child from vaccine-preventable diseases and is the national standard for health care providers who vaccinate your children.

Westgate Medication Procedures and Required Forms

  • Completed medication forms for all medications kept in the Westgate Health Office/clinic and for Westgate Field trips, including overnights.

  • All Medications need to be in the original container. Prescriptions must include the pharmacy label.

  • Controlled prescription medications must be counted and signed in and out of the clinic by a guardian.

  • Per Colorado school guidelines, homeopathic and herbal supplements may not be administered in a school setting.

  • Students may not transport medications in and out of school. A guardian must drop off and pick up all medications.

Self Carry Inhaler Authorization Forms(only recommended for older students)

Consider also having a back-up inhaler in the Health Office/Clinic in case of an emergency. Bring inhaler AND box with the prescription label to school for clinic to copy box and mark inhaler.
